Libby School District #4 REQUEST FOR QUALIFICATIONS FOR PRECONSTRUCTION PHASE SERVICES CONSTRUCTION PHASE SERVICES General Requirements 1. Project Description Libby Public School District is seeking qualified firms to undertake preconstruction and potentially construction services for HVAC upgrades that will increase efficiency, address air quality and help LPS prevent and prepare for COVID 19. Utilizing MCA 18-2-502, the intent is to provide Libby School District with the means to improve the facilities various and interrelated systems while realizing maximum life cycle value, construction coordination, utility and operational savings. 2. Eligible Design-Build Contractors Eligible design-build contractors are limited to those companies that can demonstrate successful implementation of similar projects. The contractor must also demonstrate, to the satisfaction of the Libby School District, their ability to organize, manage, and deliver a project of this type. 3. Statements of Qualification Content Statements of Qualification must be signed by an officer or principal of the firm and submitted with the information outlined in the respondent's proposal. Each RFQ response will be reviewed to determine if it is complete prior to actual evaluation. 4. Any proprietary information that the contractor does not want disclosed to the public shall be so identified on each page in which it is found. Completeness Each response will be reviewed prior to the selection process for completeness and adherence to information requested. Evaluation Process The evaluation process will objectively grade the responses on their merit and responsiveness to the needs of Libby School District. Responses will be evaluated in light of the material and substantiating evidence presented in the response; not on the basis of what is inferred. The evaluation process may include verification of references, verification of project team members, and may also include other information as directed by Libby School District B. Evaluation Criteria The Libby School District Superintendent and his designees shall evaluate each response to the RFQ based on the criteria stated below. The maximum number of points is 80.
